Scheduling an Appointment
New and existing patients may schedule an appointment by calling our office during regular business hours. You may also request an appointment through our Patient Portal. To help us prepare for your visit, we encourage new patients to download and complete our intake forms in advance from our Online Forms page.
Please have the following information available when you call to schedule: your full legal name and date of birth, your insurance card (front and back), a valid photo ID, a referral or authorization if required by your insurance plan, and any relevant imaging studies (MRI, CT, X-ray) or medical records related to your pain condition. If you have seen another pain managment or interventional pain management clinic in the past or if you are transferring care from another provider, our office can assist with obtaining your records. Records must be provided before your visit can be scheduled.
Referrals
Coastal Spine & Pain accepts referrals from primary care physicians, orthopedic surgeons, neurologists, neurosurgeons, and other healthcare providers throughout Baldwin County, Mobile County, and the Florida Panhandle and beyond. Referring providers may contact our office by phone or fax to initiate a referral. We make every effort to see referred patients promptly, and we provide timely communication back to the referring provider regarding our evaluation and treatment plan.
Insurance
Coastal Spine & Pain accepts most major insurance plans, including Medicare. If you have questions about whether we accept your specific insurance plan, please contact our office before your visit and our staff will be happy to verify your coverage. Patients are responsible for any applicable copayments, coinsurance, and deductibles at the time of service. For uninsured or self-pay patients, we offer good faith estimates in accordance with the No Surprises Act. Directions
Our office is located at 150 W. Peachtree Avenue in Foley, Alabama, conveniently accessible from Baldwin County, Mobile County, and the Florida Panhandle. We are situated near the intersection of Peachtree Avenue and Highway 59 in downtown Foley.
From Mobile: Take I-10 East to the AL-59 South exit toward Foley/Gulf Shores. Continue south on AL-59 into Foley. Turn right onto W. Peachtree Avenue. Our office will be on your right.
From Gulf Shores / Orange Beach: Take AL-59 North toward Foley. After entering Foley, turn left onto W. Peachtree Avenue. Our office will be on your right.
From Pensacola / Florida Panhandle: Take I-10 West to the AL-59 South exit toward Foley/Gulf Shores. Continue south on AL-59 into Foley. Turn right onto W. Peachtree Avenue. Our office will be on your right.
Medical Records Requests
Requests for copies of your medical records may be submitted in writing to our office. Please visit our Medical Records page for details on how to submit your request. In accordance with HIPAA regulations, all records requests require a signed authorization from the patient or their legal representative. Please allow up to 30 days for processing. Records may be sent electronically, by fax, or by mail depending on the nature of the request.
After-Hours & Emergencies
If you are experiencing a medical emergency, please call 911 or go to the nearest emergency room immediately. For urgent matters outside of regular business hours, please call our main office number and follow the prompts for our after-hours answering service. A member of our care team will return your call as soon as possible.
For non-urgent questions or appointment requests, please call during normal business hours or reach us through the Patient Portal.
