New Patients

Welcome to Coastal Spine & Pain. We understand that beginning care with a new practice can involve a number of practical questions — what to bring, what to expect, and how long the first visit will take. This page is designed to answer those questions and help you arrive prepared.

Before Your Appointment

A successful first visit depends in large part on what we are able to review before you arrive. Our team will be in contact with you in the days leading up to your appointment to gather information, verify insurance, and confirm logistics. We ask that you complete the following steps before your visit:

1. Complete the New Patient Intake Forms

Our complete set of intake forms is available on the Online Forms page. These include a general patient information form, medical history questionnaire, pain assessment, financial policy and assignment of benefits, HIPAA acknowledgment, and communication consent. Please complete these forms in advance and bring them with you to your first appointment, or arrive thirty minutes early to complete them in our office.

2. Send Your Medical Records

If you have been evaluated or treated for your current pain condition by another provider, we ask that records be sent to our office in advance of your appointment. Records may be transmitted by your previous provider directly to our office by fax to 1-888-894-5212. Particularly helpful records include:

Imaging reports and discs. MRI, CT, X-ray, and ultrasound reports of the affected area, along with the actual imaging files on disc or accessible electronically.

Prior consultation and procedure notes. Notes from previous pain management, neurosurgery, orthopedic, physiatry, or primary care visits related to your pain condition.

Operative reports. If you have had prior spine, joint, or pain-related surgery, please send the operative report.

Procedure history. Records of any prior injections, nerve blocks, radiofrequency ablations, spinal cord stimulator trials, or other interventional treatments.

Medication history. A list of current and prior medications used to treat the pain condition, including doses and the prescribing provider.

3. Verify Insurance Information

Our team will verify your insurance benefits prior to your appointment. To enable that verification, please provide your insurance information at the time of scheduling and notify us promptly if anything changes. Coastal Spine & Pain participates with most major commercial insurance plans. A list of participating insurers is available on request — please call our office to confirm coverage before your visit.

What to Bring to Your First Visit

Please bring the following with you:

Government-Issued Photo Identification

Driver’s license, state-issued identification, or passport.

Insurance Card(s)

Primary and secondary insurance cards, if applicable. Medicare beneficiaries should bring both their red-white-and-blue Medicare card and any Medicare Advantage or supplement plan card.

Completed Intake Forms

If you have completed the new patient forms in advance, please bring them with you. If not, plan to arrive thirty minutes early to complete them in our office.

List of Current Medications

A complete list of every medication and supplement you currently take, including over-the-counter medications, with the dose and frequency for each.

Imaging Discs & Records

Any MRI, CT, or X-ray discs of the affected area that have not already been transmitted to our office, along with any consultation notes or procedure reports you have copies of.

Method of Payment

For applicable copayments, deductibles, coinsurance, or balances. We accept cash, personal checks, all major credit and debit cards, and health savings account (HSA) or flexible spending account (FSA) cards.

Referral or Authorization (If Required)

Some insurance plans require a referral from your primary care provider or a prior authorization for a specialist visit. If your plan requires this, please confirm it is in place before your appointment.

A Companion (Optional)

You are welcome to bring a family member, spouse, or friend to assist you, take notes, or simply be present during your visit. We find this is especially helpful at the initial consultation.

What to Expect at Your First Visit

Your first visit is a comprehensive evaluation. Please plan for approximately sixty to ninety minutes in our office, though the visit may run somewhat shorter or longer depending on the complexity of your condition.

Check-In and Intake

Upon arrival, our front desk team will verify your identification and insurance, confirm your contact information, and ensure your intake paperwork is complete. If any forms remain unfinished, you will have an opportunity to complete them at this time.

Clinical Evaluation

A member of our clinical team will bring you to an examination room and review your history, current symptoms, prior treatments, and overall health. Dr. Webb will then perform a detailed examination focused on your pain condition, which typically includes assessment of your range of motion, strength, reflexes, sensation, and specific maneuvers that help localize the source of pain.

Review of Imaging and Records

Dr. Webb will personally review your imaging studies and prior records as part of the evaluation. In many cases, he will review the images with you and explain the relevant findings.

Discussion and Treatment Plan

After the examination, Dr. Webb will discuss his clinical impression with you, explain what is likely driving your pain, and outline the treatment options that may be appropriate. Coastal Spine & Pain is an interventional pain management practice, and treatment recommendations will generally reflect a multimodal approach — which may include interventional procedures, non-opioid medication management, physical therapy, behavioral health support, or referrals to other specialists as appropriate.

You will have a full opportunity to ask questions and discuss the recommended plan. No procedural treatment will be performed at the first visit without your understanding and consent.

Scheduling and Follow-Up

Before you leave, our team will schedule any recommended follow-up visits, procedures, or imaging studies, and will work with your insurance to obtain any necessary prior authorizations.

Important Notes for New Patients

Independent clinical evaluation. Every new patient receives a comprehensive, independent clinical evaluation. Prior diagnoses, prior prescriptions, and prior treatment recommendations are reviewed but do not determine our treatment plan. Our treatment plan is based on our own examination, the current standard of care, and applicable regulations.

Scope of practice. Coastal Spine & Pain provides interventional pain management. We do not provide medication-assisted treatment (MAT) for opioid use disorder, and we do not transition patients between MAT and traditional opioid therapy. Patients currently receiving MAT should continue care with their established MAT prescriber. For more information, please review our Medication Management page.

No specific treatment is guaranteed. Scheduling an appointment does not guarantee any particular treatment, medication, procedure, or prescription. All treatment decisions are made at the clinical discretion of the physician following evaluation, in accordance with applicable state and federal law.

Financial & Policy Matters

Insurance and Payment

Copayments and any patient-responsibility balances are due at the time of service. We will file claims with your insurance on your behalf as a courtesy and will work with you to understand any portion of the visit that is not covered. Patients without insurance may be seen on a self-pay basis — please contact our office in advance to discuss self-pay rates.

Appointment Confirmations

You will receive a confirmation of your appointment by phone, text, or email in the days leading up to your visit, depending on the communication preferences you have indicated. Please respond promptly to confirm.

Cancellations and Rescheduling

If you are unable to make your appointment, please notify our office at least twenty-four hours in advance so the time slot can be offered to another patient. Repeated late cancellations or no-shows may affect future scheduling.

Privacy

Address. Coastal Spine & Pain is located at 150 W. Peachtree Avenue, Foley, Alabama 36535, inside Peachtree Medical Center.

Office Hours. Monday through Thursday, 8:00 AM to 4:30 PM. Closed Friday, Saturday, and Sunday.

Parking. Free patient parking is available in the lot adjacent to the medical center. Accessible parking is located near the main entrance.

Accessibility. The building and our office suite are wheelchair accessible. If you require additional accommodation, please notify our team in advance and we will be happy to assist.
